City of Flat Rock issued the following announcement on Sept 21.
Flat Rock Resident Public Health Survey
The Michigan Department of Health and Human Services (MDHHS) is conducting this survey to collect information about people who live in the City of Flat Rock, Michigan. This is in response to the release of gasoline from the Flat Rock Ford Assembly Plant into the City of Flat Rock sanitary sewer system.
We would like to know about odors and health issues that you, or anyone from your household, experienced. The survey is online and should take about 15 minutes to complete.
Only one adult, 18 years or older, from your household should answer this survey. This means that one person will answer the survey on behalf of all people that live in the household, including family and non-family members.
We only want one survey per household, so please only complete the survey once.
All the information you provide on this survey will be kept confidential and will be securely stored. We will protect the information you share with us to the fullest extent allowed by law. You can choose not to answer any question that you do not want to.
